The Keystone Case Management Program helps people in challenging circumstances to maintain their stability and independence. Keystone community social workers help clients find and connect to resources and services and solve problems that threaten self-sufficiency.
The program assists seniors and adults with disabilities through several home- and community-based Medical Assistance waivers. Click here for information about eligibility.
Keystone's new program is Keystone Care Consultants, a private-pay service which helps seniors and their families find the best care options for their situation.
