The Keystone Case Management Program helps people in challenging circumstances to maintain their stability and independence. Keystone community social workers help clients find and connect to resources and services and solve problems that threaten self-sufficiency.
The program assists seniors and adults with disabilities through several home- and community-based Medical Assistance waivers. Click here for information about eligibility.
Keystone also provides school-linked community social work for families who live in the White Bear Lake or North St. Paul/Maplewood/Oakdale school district community. Click here for information about this program.
Keystone's new program is Keystone Care Consultants, a private-pay service which helps seniors and their families find the best care options for their situation. Our skilled social workers understand the full continuum of needs — from maintaining wellness and self-sufficiency, to living with acute and chronic health conditions, to managing end-of-life care, drawing on their experience and knowledge of resources in developing recommendations tailored to each situation.
