Introducing Keystone Care Consultants

Keystone Community Services has launched a new program for families concerned about the well-being of an aging parent or relative. Specializing in care options for seniors, Keystone Care Consultants is a team of skilled social workers who assess the needs of the senior, develop a comprehensive plan to support their optimum independence and welfare, and coordinate selected services as needed."Our focus is on maintaining the well-being of seniors and the peace of mind of those who care about them," said Keystone president Eric Nyberg. "Since each family and situation is unique, we help families determine the best course of action as a family member ages and needs more support."

Nyberg said the program, a private-pay service, is an offshoot of Keystone's Case Management Program for seniors, disabled adults and families. "Our staff is very knowledgable and highly experienced in coordinating care for elderly and disabled adults — from maintaining wellness and self-sufficiency, to living with acute and chronic health conditions, to managing end-of-life care," said Nyberg.
