Overcoming obstacles to independence
To remain living
independently, frail elderly and low-income adults with functional
limitations such as hearing or vision loss must negotiate a very
complex system of finance, state and federal regulations and services.
Keystone professional case managers and trained volunteer peer counselors
assist seniors in locating and accessing needed services and resources.
- PROGRAM SUMMARY
- • Case management provides
services to homebound seniors referred by Ramsey County who are
at risk of placement in an institutional setting. Case managers,
who are licensed social workers, coordinate community efforts,
work with caregivers and provide periodic reassessments, authorizations
for services and care coordination of in-home services.
• Trained senior peer counselors visit
homebound elderly, providing information and referral, and assistance
with service forms. Regular visits from these volunteer counselors
offer vital social interaction for otherwise isolated residents.
Peer counselors also provide a variety of informal services and
serve as a "first alert" system as needs of the homebound
individual change over time.
